簡(jiǎn)要規(guī)則描述：為了更好地滿足老年人居家照料需求，切實(shí)做好居家養(yǎng)老服務(wù)工作，減輕家庭照護(hù)負(fù)擔(dān)，設(shè)立高行鎮(zhèn)（周橋東溝森蘭片區(qū)、華高高行片區(qū)、綠洲片區(qū)）居家養(yǎng)老服務(wù)項(xiàng)目，為符合市、區(qū)兩級(jí)居家養(yǎng)老服務(wù)補(bǔ)貼政策的老年人提供居家上門服務(wù)。具體內(nèi)容和相關(guān)要求詳見(jiàn)《采購(gòu)需求》。
Brief Specification Description:In order to better meet the home care needs of the elderly, effectively carry out home-based elderly care services, and reduce the burden of family care, a home-based elderly care service project has been established in Gaoxing Town (Zhouqiao Donggou Senlan Area, Huagao Gaoxing Area, and Luzhou Area), providing home visit services for the elderly who meet the home-based elderly care service subsidy policies at both the municipal and district levels. For specific content and relevant requirements, please refer to the "Procurement Requirements".
合同履約期限：自合同簽訂之日起 1 年（****年4月1日—****年3月**日）
The Contract Period: 3、根據(jù)《上海市政府采購(gòu)供應(yīng)商登記及誠(chéng)信管理辦法》 已登記入庫(kù)的供應(yīng)商。
4、其他資格要求：
1）供應(yīng)商與采購(gòu)人、其他潛在的供應(yīng)商等本項(xiàng)目相關(guān)單位不存在利益關(guān)系。單位負(fù)責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同供應(yīng)商，不得參加同一合同項(xiàng)下的政府采購(gòu)活動(dòng)。
2）法人依法設(shè)立的分支機(jī)構(gòu)以自己的名義參與投標(biāo)時(shí)，應(yīng)提供依法登記的相關(guān)證明材料和由法人出具的授權(quán)其分支機(jī)構(gòu)在其經(jīng)營(yíng)范圍內(nèi)參加政府采購(gòu)活動(dòng)并承擔(dān)全部民事責(zé)任的書(shū)面授權(quán)。法人與其分支機(jī)構(gòu)不得同時(shí)參與同一項(xiàng)目的采購(gòu)活動(dòng)。
3）參加本次政府采購(gòu)活動(dòng)前三年內(nèi)，在經(jīng)營(yíng)活動(dòng)中沒(méi)有重大違法記錄。
4）本項(xiàng)目?jī)H面向小、微型企業(yè)采購(gòu)。
(c)Specific qualification requirements for this program:3. Suppliers that have been registered and included in the database in accordance with the "Shanghai Government Procurement Supplier Registration and Integrity Management Measures". 4. Other qualification requirements: 1) The supplier has no interest relationship with the purchaser, other potential suppliers or other units related to this project. Suppliers whose legal representatives are the same person or who have direct controlling or management relationships shall not participate in the government procurement activities under the same contract. 2) When a branch of a legal person established in accordance with the law participates in the bidding in its own name, it shall provide relevant proof materials registered in accordance with the law and a written authorization issued by the legal person authorizing its branch to participate in government procurement activities within its business scope and bear all civil liabilities. A legal person and its branches shall not participate in
